This community plan reflects the journey of Te Wāhi Tiaki Tātou since it was first established in 2022. Its’ purpose is to share the aspirations and priorities of the Porirua community. It is a high-level document that outlines a pathway for the transformation of hauora in our rohe and sets out on a journey to change the way our health system operates based on the voices and needs of our whānau and community.
